Request for Consent You are being invited to participate in a research study titled “Caring Science, Mindful Practice Open Online Course” being conducted by Kathleen Sitzman, PhD, RN, CNE, a Professor at East Carolina University in the Nursing department. The goal is to survey participating individuals in the Caring Science, Mindful Practice Open Online course. The survey will take approximately 5 minutes to complete. It is hoped that this information will assist us to better understand student attributes, needs, and experiences in the open online learning setting. The survey is anonymous, so please do not write your name. Your participation is voluntary. You may choose not to answer any or all questions, and you may stop at any time. There is no penalty for not taking part in this study.
